Provider Registration

If you are a Care Provider (Assisted Living Community, HUD Housing Facility or someone that provides Care Services such as a Guardian) and your name is not on the Gift Tag Entry form as a Provider, then please submit your information to be put on the list. You will be contacted with instructions on Participation. The information entered here will be used to contact you when Gift Bags are ready for Pick-up.
